
The CHFS Department of Medicaid Services provides Medicaid Waivers to provide in-home care and community support services to allow individuals that need nursing level of care to remain in the home. Services include Case Management, Minor Home Adaptation, Adult Day Healthcare, homemaking, personal and attendant Care, goods and supplies.
KIPDA can assist with initiating applications for individuals with limited internet access or other barriers keeping them from applying on their own behalf. KIPDA does not make any determinations of eligibility or approval.
Kentucky operates six Home and Community Based Services (HCBS) waiver programs. These programs cover services for individuals who are aged or have a disability and require long-term support to remain living at home or in the community.
Individuals can initiate their own application for a waiver through the Kynect website.
If you have already received notice from the Department of Medicaid Services that you’re approved and granted a spot on a waiver program, you will need to choose a case management provider within 60 days. Many providers will have waitlists. A directory can be found below:
KIPDA provides PDS Case Management for clients that have already been approved and granted a spot on the waiver by the CHFS Department of Medicaid Services.
