Nutrition Assessor — Responsible for telephone screening, walk-in screening, and/or assessments of individuals to determine program eligibility for appropriate long-term care options and nutrition services.
Homecare Assessor and Case Manager — Responsible for performing eligibility assessments for in-home services for older persons and ensuring in-home services are delivered as approved in the Plan of Care.
