Support Broker -
Position is responsible for performing eligibility assessments for in-home services for older adults and persons with disabilities; performs related duties as required.
General Duties and Responsibilities: Assist the In-Home Services Coordinator, Division Director and any outside agency with the development of the Consumer Directed Options (CDO) Program; act as the Support Broker for the CDO clients; Perform in-home eligibility assessments; perform in-home reassessments; monitor clients’ monthly with home face-to-face visits; negotiates and prepares care plans, input plans into computer system; links client with proper resources to solve problems; works with providers/vendors; schedules and checks on services; determines cost-sharing amounts; refers clients to other agencies, as appropriate; involves family/friends/client in care planning; contacts providers/vendors to arrange for services; initiates any correction action; i.e. modify care plan, reassess, or terminate services; initiates complaint forms and/or help resolve client complaints with appropriate staff; trains client and client’s employees; processes clients’ employees’ timesheets; files case notes as needed; rerforms other duties as required.
DESIRABLE QUALIFICATIONS
Training and Experience: Bachelors Degree in social work, sociology or related field with a minimum of two years professional experience in working with appropriate populations, or a Masters in social work, sociology or related field with six months professional experience while employed at a home health agency, long-term care facility, public health agency, or social services agency.
Salary - $30,000 annually
