| The following programs and services are available to persons with disabilities, older adults and caregivers in the following counties: Bullitt, Henry, Jefferson, Oldham, Shelby, Spencer and Trimble. |

SHIP provides information, counseling and assistance to persons regarding Medicare, Medicaid, Social Security, Prescription Assistance, Food Stamps, etc. The service is provided at no charge. SHIP's focus is to educate and assist persons in making informed decisions about their health care, based on individual circumstances and needs.

The Family Caregiver Program provides information and assistance to caregivers; counseling, training and support groups; respite for caregivers; workplace education and training; and assistance with supplemental services such as home adaptation or medical supplies. The basic criteria for eligibility for the program are:
- You are providing care for someone 60 years or older or providing care for and individual under 60 with early on-set dementia,
- You are a grandparent or relative who is 55 years or older caring for a child that is 18 and under,
- You are 60 years or older caring for an idividual with Mental Retardation or Developmental Disabilities.

This program assists persons over 60 years of age who are either at risk of becoming institutionalized or who are already in such a facility but could live at home with assistance. The goal is to maintain an elderly resident in his or her home by providing needed assistance and case management. Services include home delivered meals, homemaking, personal care, escort, chore, and respite. Acceptance to the program is based on assessment results; fees are sliding scale based on the client's income.

Adult Day Care is a structured program that is designed to keep older persons active, safe, stimulated and entertained. Eligibility is based on assessment results with a sliding scale fee. Attendance is individualized to the participant's needs.

A nursing home ombudsman advocates on behalf of nursing home residents by investigating complaints and informing residents of their rights through volunteer visits.

Title III funding allows KIPDA to create opportunities for seniors to access transportation, advocacy and outreach, home delivered meals and congregate meals, health promotion education, financial management, legal assistance, counseling, and more. These services are delivered through contracts with local agencies.

Each center is unique in its programming. At a minimum a visitor can expect to find the following services and activities: educational speakers covering numerous topics, congregate meals, nutrition counseling, health and fitness programs, recreation and socialization. Many centers offer transportation to and from the center. Most centers also offer home delivered meals for the more frail elderly who cannot get to the center.

The KIPDA Nutrition Program for the Elderly is funded through the Older Americans Act (Federal Title III-C), Kentucky State General and local community funds. The program provides lunch in group settings and in the homes of fragile older adults. All meals served in the program supply at least one-third of the recommended daily need for nutrients and are intended to improve dietary intake for anyone over 60 years or older.
Nutrition services are free to persons 60 years or older and their spouse of any age. Disabled dependents may also be eligible. The program asks for donations from participants and the community at large and uses the income to provide more meals.

Established by the Kentucky General Assembly, this program mandates personal care attendant services for severely physically disabled adults age 18 and over. The client must be mentally alert with permanent, temporary or recurring functional loss of two or more limbs. The disabled adult has the management responsibility of hiring, supervising and dismissing the attendant. Services allow the adult to achieve greater physical independence.

Kentucky Caregiver Program (KCP)
KIPDA Grandparents Raising Grandchildren Program

For grandparents who have the primary responsibility of raising their grandchildren the KCP program offers both supportive and supplemental services. Supportive Services which include: information, assistance, counseling, support groups, training and access to community resources. Supplemental Services are provided through a voucher system to assist with the purchase of school supplies, clothing, legal services, respite services, medical and dental services, and other needed services as authorized by KIPDA. The basic eligibility criteria are as follows:
- Total household income must be at 150% of poverty or below
- Grandchild must not be a recipient of Kinship Care
- Parents of the Grandchild must not reside in household

CDO allows eligible Medicaid Waiver members to choose their own providers for non-medical waiver services. CDO is being offered for Kentucky Medicaid Waiver members who currently receive services or who become eligible through Home and Community Based Waiver (HCB).

Disease Prevention and Health Promotion

Disease prevention and health promotion services are designed to help elders with chronic disease or at high risk for developing chronic diseases. The projects help senior centers, nutrition sites and other agencies make prevention a reality in the everyday lives of older adults. KIPDA sponsors AARP Driver Safety Programs, walking programs, Arthritis Association Exercise classes, Living Well Workshops, healthy cooking classes, and Regional and State Senior Games. We help seniors quit smoking and manage their medications better.
